Egypt\u2019s Medical Placement Cuts"},"content":{"rendered":"<p>Egypt\u2019s medical-graduate placement crisis is escalating as the Health and Population Ministry replaces automatic government appointments for all dentistry, physiotherapy and pharmacy graduates with placements based on facilities\u2019 stated needs. Meanwhile, the residency round expected in December 2025 remained delayed.<\/p>\n<p>On February 21, the ministry <a href=\"https:\/\/www.facebook.com\/egypt.mohp\/posts\/%D8%A5%D8%B9%D9%80%D9%84%D8%A7%D9%86%D8%AA%D8%B9%D9%84%D9%86-%D9%88%D8%B2%D8%A7%D8%B1%D8%A9-%D8%A7%D9%84%D8%B5%D8%AD%D8%A9-%D9%88%D8%A7%D9%84%D8%B3%D9%83%D8%A7%D9%86-%D8%B9%D9%86-%D8%A8%D8%AF%D8%A1-%D8%AD%D8%B1%D9%83%D8%A9-%D8%A7%D9%84%D8%AA%D9%86%D8%B3%D9%8A%D9%82-%D9%84%D8%AA%D9%83%D9%84%D9%8A%D9%81-%D8%AE%D8%B1%D9%8A%D8%AC%D9%89-%D9%83%D9%84%D9%8A%D8%A7%D8%AA-%D8%A7%D9%84%D8%B9%D9%84%D8%A7%D8%AC-%D8%A7%D9%84%D8%B7\/1252359683741910\/\">announced online coordination<\/a> for physiotherapy graduates of the 2023 class, including both examination rounds, for posts in ministry units and other bodies. Distribution would consider grades, geography and hospital need.<\/p>\n<p>The policy affecting the 2023, 2024 and 2025 cohorts prompted opposition from professional syndicates and MPs. Graduates describe the change as retrospective and a threat to their futures, demanding whole-cohort placement and clear, fair workforce-allocation criteria.<\/p>\n<p>About four years earlier, the Higher Placement Committee had retained placements for medical, nursing and health-sciences graduates from the 2022 class onward according to actual ministry needs, starting with the 2025 placement round, while rejecting complete abolition and maintaining <a href=\"http:\/\/mhealth.cu.edu.eg\/Atbaa\/Atbaa2020TazlloumResult\/%D9%82%D8%A7%D9%86%D9%88%D9%86%20%D8%B1%D9%82%D9%85%2029%20%D9%84%D8%B3%D9%86%D8%A9%201974.pdf\">Law No. 29 of 1974<\/a>.<\/p>\n<p>In August 2025, the ministry announced <a href=\"http:\/\/mhealth.cu.edu.eg\/Niabat\/E3lanat_Niabat_May_2025\/%D8%A7%D8%B9%D9%84%D8%A7%D9%86%20%D8%AD%D8%B1%D9%83%D8%A9%20%D9%86%D9%8A%D8%A7%D8%A8%D8%A7%D8%AA%20%D9%85%D8%A7%D9%8A%D9%88%202025.pdf\">conditions for the May 2025<\/a> physician-residency round, covering December 2023 entrants and earlier cohorts. Applicants needed a full year of actual service for regular specialties or six months for urgently needed specialties. Registration through the doctors\u2019 placement website within the specified window was required to avoid being treated as relinquishing a residency.<\/p>\n<p>In September 2023, the ministry\u2019s General Department of Placement Affairs approved completion of amended placements for physicians, pharmacists, dentists and other medical categories where an amendment decision was issued before the two-year placement ended but could not be implemented in time. He argues that reduced placements do not reflect demand, citing a population approaching 120 million and only around 18,000 physiotherapists. These are his figures in the report. Physiotherapy is integral to orthopaedic and neurological injuries, post-operative rehabilitation, sports injuries, chronic conditions and older people\u2019s care, he says.<\/p>\n<blockquote>\n<p>\u201cMany villages and remote areas lack enough physiotherapy units, forcing patients to travel to other governorates,\u201d he told Zawia3. Repeated sessions over weeks or months make those journeys a major burden.<\/p>\n<\/blockquote>\n<p>Placing graduates could fill gaps and distribute care more fairly, Khaled argues. Physiotherapy is part of patients\u2019 right to treatment, rehabilitation and restored mobility, rather than a medical luxury.<\/p>\n<p>The General Physiotherapy Syndicate appealed to Prime Minister Mostafa Madbouly for placement of all 2023 graduates after the committee allocated only 1,366 posts. It warned that changing rules without a transition threatens investment in private, national and international higher education and thousands of graduates\u2019 futures, while university hospitals, primary-care and sports-medicine units face specialist shortages.<\/p>\n<p>The syndicate requested a three- to five-year delay before needs-based placement, a joint committee of the health and higher-education ministries and Parliament to assess labour-market demand, a halt to licences for new colleges and admissions aligned with jobs and training.<\/p>\n<p>Sara Maher, a 2023 graduate of a public pharmacy faculty, says her class was the last to study under the old system, entering on the understanding that government placement followed graduation. Applying needs-based rules now is retrospective, she argues.<\/p>\n<p>She told Zawia3 that her cohort should have entered the 2024 placement round. Initial discussion concerned later rounds, leaving graduates abruptly subject to a decision they did not anticipate. They are not responsible for the expansion of private and national colleges or growing graduate numbers, she says.<\/p>\n<p>After long, demanding studies intended to secure stable work and a dignified livelihood, Sara and colleagues waited roughly three years hoping for placement. She urges the government to reconsider and appoint the whole class.<\/p>\n<p>The <a href=\"https:\/\/www.instagram.com\/p\/DVINLdtjH43\/\">General Pharmacists Syndicate<\/a> similarly said the 2023 class waited about three years, contrary to its interpretation of Article 2 of Law No. 29 of 1974, which it says requires placement within a year. The delay harmed professional prospects and stability.<\/p>\n<p>The syndicate rejected placement of just 45% of the cohort. Health institutions advertising contract-based pharmacist posts demonstrate real demand, it argues. It requested full placement and reconsideration of college expansion according to national needs.<\/p>\n<p>Amr Mohamed, a 2023 dentistry graduate, describes three years of open-ended uncertainty. He seeks full placement for the 2023 and 2024 cohorts, arguing that they entered a stable system without warning of a fundamental policy change. A harmful new decision should not be applied retrospectively, he says.<\/p>\n<blockquote>\n<p>\u201cIf the state considers graduate numbers a crisis, why did it keep opening private and national colleges without controlling admissions or raising entry thresholds?\u201d he asks. Students should not bear the consequences of poor planning after years of study and expense. \u201cWas our mistake working hard, studying and waiting for a fair chance to work in our country?\u201d<\/p>\n<\/blockquote>\n<p>In his home governorate of Luxor, some health units lack dental facilities altogether, others have equipment without dentists and some sites have more dentists than needed, Amr says. He sees poor distribution and management as the core issue. Prosecutors accused them of spreading false news and joining a banned group and ordered pretrial detention following legal and solidarity activity opposing placements reduced to around 40%. These are reported charges, not findings of guilt.<\/p>\n<p>Rights lawyer Malek Adly, director of the Egyptian Center for Economic and Social Rights, argues that security responses worsen professional crises. Challenging an administrative decision and demanding employment rights fall within freedom of expression, organisation, syndicate activity and access to courts.<\/p>\n<blockquote>\n<p>Pretrial detention in such cases damages professional freedoms and sends intimidating signals that impede discussion of health policy, Adly told Zawia3. The system needs open dialogue about its problems.<\/p>\n<\/blockquote>\n<p>He sees the placement controversy and delayed December 2025 residency round as a transparency crisis. Without clear data on medical staff, geographic distribution, facilities and beds relative to population, claims of surplus or shortage are hard to assess.<\/p>\n<p>Claims of surplus conflict with repeated reports of inadequate services and continuing physician emigration, Adly says. Losing medical expertise poses a strategic risk to national health security.<\/p>\n<p>The state must manage health workers efficiently while respecting commitments to staff and patients, he argues. Public healthcare should not follow a purely profit-driven logic, particularly under citizens\u2019 economic pressures. Their concerns cover dentistry, pharmacy, physiotherapy and physician <a href=\"https:\/\/www.albawabhnews.com\/5323307\">residencies<\/a>.<\/p>\n<p>Among them were Rawya Mokhtar, Irene Saeed, Nevine Iskander, Freddy al-Bayadi, Safinaz Talaat, Hanaa Anis Rizkallah, Hossam Khalil, Mohamed al-Dematy, Sara al-Nahhas and Donia Hany Abdel Aziz. They sought transparent allocation, explicit needs criteria, fair distribution, protected professional rights and a coherent workforce plan.<\/p>\n<p>Irene Saeed, a House Health Committee member and head of the Reform and Development Party\u2019s parliamentary bloc, criticised the ministry for continuing percentage-based placement before discussing MPs\u2019 requests. An urgent Health Committee session was needed given the timing and consequences, she says, but the requests had not yet reached its agenda.<\/p>\n<p>She told Zawia3 she fears insufficient transparency and equal opportunity. Signs of additional demand conflict with later ministry recruitment on contracts, raising questions about workforce management and potential waste, since contracted posts may be costlier and less stable than placements.<\/p>\n<p>College regulations incorporating placement encouraged families to pay substantial tuition in expectation of government employment, Saeed says. Reductions without prior coordination with higher education leave graduates facing unexpected circumstances, unemployment or limited private-sector openings, particularly in pharmacy. Emigration is an alternative not equally available to everyone, especially women, she adds.<\/p>\n<p>Saeed also questions grade-based selection across universities with different assessment practices. Grades may not reliably reflect professional competence. Sudden changes require clear reasons and published studies. She criticises the lack of disclosed legal grounds, methods for choosing percentages and evidence of actual surplus.<\/p>\n<blockquote>\n<p>Mokhtar told Zawia3 she requested a published planning study explaining the percentages, regional health-unit shortages, five years of graduate numbers compared with retirements and resignations, funded positions, consistency with Law No. 29 of 1974 and whether fiscal considerations drove the reduction.<\/p>\n<\/blockquote>\n<p>She sees inconsistency between government claims that no shortage exists and continuing college expansion, questioning coordination between health, higher education and labour ministries before admissions increase.<\/p>\n<p>Mokhtar fears further physician emigration and expressed concern about detained 2023 dentistry and pharmacy graduates. She calls for a clear, fair approach balancing public needs and graduates\u2019 rights while preserving system stability.<\/p>\n<p>Nevine Iskander says the crisis predates this year, developing alongside private and national university expansion and rising graduate numbers. Needs-based placement has featured in previous parliamentary discussions.<\/p>\n<p>She argues that the nearly three-year delay breached the one-year placement requirement and left graduates without a clear career path.<\/p>\n<p>The 2023 and 2024 classes entered under long-established full-placement rules, she says. She recalls a 2020 ministry announcement that needs-based placement would apply to students entering that year, arguing earlier entrants should retain previous rules. Changing those expectations reveals management failures in her view.<\/p>\n<p>Parliament received complaints from graduates nationwide, Iskander says. MPs suggested staged placement rounds instead of excluding parts of cohorts and asked how equal treatment would be ensured. Pharmacists increased from 51,286 to over 103,000, against stated demand of 3,525, and physiotherapists from 8,808 to 18,411, against demand of 1,026. The main reductions affect dentistry, pharmacy and physiotherapy, where the ministry cites numbers beyond existing needs and facility capacity. He sees a fundamental lack of integrated admissions planning.<\/p>\n<blockquote>\n<p>\u201cThere are no published figures showing the actual distribution map,\u201d Amin told Zawia3\u2014whether staffing relative to equipment or geographical allocation. Debate lacks precise, transparent data for assessing a real surplus or shortage.<\/p>\n<\/blockquote>\n<p>Reducing or delaying placement is not simply about losing a job, especially given low salaries, he argues. Graduates lose systematic training. Unplanned expansion risks disguised unemployment, weaker competence, migration and later difficulties obtaining recognition abroad.<\/p>\n<p>Placement provides supervised practical experience within government hospitals under senior clinicians. Without it, graduates may practise outside structured training, affecting service quality and patient safety, Amin warns. Planning must connect admissions, medical education, demand and protected training pathways.<\/p>\n<p>Dr Ayman Attia accepts that needs-based staffing may have economic justification but objects that students enrolled under a decades-old placement system without advance notice of change.<\/p>\n<p>He told Zawia3 that the consequences extend beyond employment to clinical training under experienced practitioners, for which government institutions are a principal post-graduation route.<\/p>\n<blockquote>\n<p>Excluding large numbers can deprive them of meaningful experience, Attia says. Private hospitals are not an easy substitute because they tend to recruit people already trained in government or university settings.<\/p>\n<\/blockquote>\n<p>Without structured training, graduates\u2019 careers and the quality and safety of healthcare may suffer over the long term, he warns.<\/p>\n<p>Economic limits on absorbing every graduate may lie behind the policy, Attia says. Addressing them requires advance planning and policies announced years before implementation so students can make informed decisions.<\/p>\n<p>President Abdel Fattah al-Sisi has <a href=\"https:\/\/www.cairolive24.com\/2214350\">called for reviewing<\/a> study programmes that no longer meet labour-market needs and making clear decisions about fields without genuine employment opportunities, to avoid students spending years without professional returns.<\/p>\n<p>The placement dispute reveals a deeper gap in health-workforce planning and coordination between education, healthcare and employment institutions. Graduates fear unemployment or migration while the government insists on actual needs.
